PHYSICAL EXAM, DENTAL, VISION & IMMUNIZATION REQUIREMENTS
Physicals that are turned in over the summer are to be placed in the RED DROP BOX located in the foyer
outside the nurse’s office. Please do not turn physicals into the grade school, high school, or district office.
SCHOOL PHYSICALS are required for entrance into Pre-K, Early Childhood, Kindergarten, 6th and 9th
grade. Physicals are required to be on file by October 15, 2024.
SPORT PHYSICALS are required prior to participating in all sports. Jr. High and High School sport
physicals must be less than 395 days old (13 months) and must remain current throughout the entire
season. Sport physicals for fall sports that begin before school starts should be turned in 1 week
prior to the first practice (sport physicals turned in over the summer must be placed in the RED
DROP BOX located in the foyer outside the nurse’s office).
VISION EXAMINATIONS are required for students entering Kindergarten. The deadline to turn in vision
exams is October 15, 2024.
DENTAL EXAMINATIONS are required for students in Kindergarten, 2nde, 6th and 9th grade. The
deadline to turn in dental exams is May 15, 2025 (at the end of the school year).
LEAD SCREENING is a requirement for PreK, Early Childhood, and Kindergarten.
IMMUNIZATION are required to be up to date and current at all times for all grade levels including PreK
and Early Childhood. A complete list of immunizations requirements is available from the school nurse.
BIRTH CERTIFICATES must be on file at the school upon entering Kindergarten. A certified birth certificate
from the county is required (hospital certificates are not acceptable).
Attendance at Pawnee School District #11 is subject to compliance with immunization and physical exam
requirements set forth in the Illinois School Code 105 ILCS 5/27-8.1
